Meet Dr. Carlos Ayala
Double board-certified otolaryngologist & facial plastic surgeon
For Dr. Ayala, medicine has always been about service.
Long before opening his practice in South Texas, he built his career around caring for people in moments when they needed it most. From treating injured service members during wartime to helping launch a medical school in an underserved region of Texas, his path as a surgeon has consistently been shaped by a simple philosophy: use your skills where they can make the greatest difference.
Today, Dr. Ayala brings that same commitment to every patient he sees, combining elite surgical training with an approach that emphasizes trust, natural results and thoughtful, personalized care.

From the Bronx to Medicine
Dr. Ayala grew up in the Bronx, during a time when the neighborhood faced significant economic and social challenges. Raised by a single mother who worked tirelessly to support her family, he learned early on that hard work and determination were the only paths to success.
Determined to build a future through service and education, Dr. Ayala joined the United States military after high school. Over the years, that decision would lead to scholarships that supported his medical training, taking him from a nurse corps Navy officer, to chief otolaryngologist and lieutenant colonel in the Air Force, to a facial plastic surgeon who has served patients with diligent care, both in wartime and in the community he now calls home.
His medical education began at UCLA, where he distinguished himself quickly and helped create Spanish classes for medical students to overcome language barriers in patient care. He graduated at the top of his class and was inducted into Alpha Omega Alpha (AOA), a prestigious honor society that recognizes academic excellence and leadership in medicine.
It was during his time at UCLA that he became involved in research and academic work, eventually co-authoring educational materials for medical students with Nobel Laureate Louis Ignarro, PhD. Dr. Ignarro would also write Dr. Ayala’s recommendation for Harvard’s otolaryngology residency program — one of the most competitive in the country — to which he was accepted.
After completing his education and residency training, Dr. Ayala returned to military service as a board-certified otolaryngologist, where his surgical skills would soon be tested in some of the most demanding clinical environments imaginable.













Surgery on the Front Lines
Following his residency training at Harvard, Dr. Ayala resumed active duty, serving as an ENT specialist with the United States Air Force in Germany. His work included taking care of wounded troops arriving from Iraq and Afghanistan. He also had the opportunity to support NASA as part of the medical standby team, receiving specialized training to assist astronauts at alternate landing sites if a mission needed to abort.
While in Germany, Dr. Ayala saw many soldiers coming back from deployment with severe facial trauma and recognized a growing need for surgeons who could treat these injuries. Wanting to use his skills and knowledge to help, he applied for a facial plastic surgery fellowship in Beverly Hills, further refining his expertise in complex procedures involving the face.
After completing this post-residency fellowship training and becoming board-certified again, this time in facial plastic and reconstructive surgery, he was deployed to Afghanistan, where he served as a facial plastic surgeon at Bagram Airfield.
“At the time, I was the only facial plastic surgeon in the country taking care of soldiers and casualties,” he recalls. There, he treated servicemembers with severe facial injuries, as well as locals with congenital differences and traumatic wounds, applying many of the same reconstructive principles to help military personnel and civilians alike.
In 2013, Dr. Ayala retired from the Air Force, concluding an impressive military career that spanned over two decades and earned him numerous awards and accolades, including a letter of appreciation from the President of the United States for distinguished service.
Serving the Rio Grande Valley
After retiring from the military, Dr. Ayala had many options for where to establish his practice, Ayala ENT & Facial Plastic Surgery. Instead of choosing a major cosmetic surgery hub, he chose to help develop medical services in the southernmost region of Texas near the U.S.–Mexico border: McAllen.
Here, he recognized an opportunity to make a meaningful impact by bringing advanced specialty care to a region where patients previously had to travel long distances to find it.
Since his arrival more than a decade ago, Dr. Ayala has helped establish a new medical school in the region, the University of Texas Rio Grande Valley (UTRGV), and served as Clinical Professor of Otolaryngology-Head & Neck Surgery at that same school. As a bilingual physician fluent in both English and Spanish, he has also provided specialized medical care to a historically underserved population. In addition, he has extended his impact beyond the region through volunteer work with the Interface Kids organization in Mexicali, Mexico, where he focuses on providing cleft rhinoplasty care to children in need. And over the years, he has become a trusted member of the community.
Many patients first learn about his practice through referrals from friends, family members or other physicians. That local trust is something he values deeply.
“This is a small community,” he says. “You’re going to see me in church on Sunday. You’re going to see me in the supermarket. I live here, too.”

The Nose Expert™
Dr. Ayala chose to specialize in otolaryngology (ear, nose and throat surgery) due to the technical challenge it presented. “ENT is one of the most difficult specialties to get into,” he says. “It’s competitive. The anatomy is complex.”
He was drawn to facial plastic surgery and rhinoplasty for the same reason. The face presents a unique challenge for surgeons, where precision is essential and even the smallest changes can have a significant impact. “You can’t hide your face,” he explains.
Today, as a double board-certified otolaryngologist and facial plastic surgeon, his practice combines these two closely related specialties. This dual background allows him to effectively treat both the form and function of the nose, an advantage to patients who may be struggling with breathing or sinus issues and cosmetic concerns they’d like to address.
His expertise has earned him a reputation as The Nose Expert™. As the first double board-certified otolaryngologist and facial plastic surgeon in the Rio Grande Valley, Dr. Ayala can treat the nose in ways most surgeons cannot.
Rhinoplasty and sinus surgery, including his own Simply Sinus Procedure™, are among those he performs often, along with blepharoplasty (eyelid surgery), otoplasty (ear surgery), nasal reconstruction and skin cancer removal. “These are what I find most rewarding,” he says. “They allow me to help patients breathe freely and step back into the world with a renewed sense of confidence and health.”
A Philosophy of Natural Results
No matter what procedure he is performing, he approaches each with a clear philosophy: every patient should still look like themselves.
He does not believe in applying a single aesthetic standard to every face. During consultations, he works closely with his patients to understand what they would like to improve and to set realistic expectations. In some cases, that means guiding patients away from trends or results that may not suit their features.
“No two noses are the same,” he explains. “Each surgery is planned around that individual person’s anatomy and goals, and with ethnic and cultural preservation in mind.”
When he is treating sinus problems, allergies, nasal obstruction and other conditions that affect breathing, he takes the same personalized approach, carefully evaluating the underlying cause of a patient’s symptoms and developing a treatment plan that restores comfort, airflow and long-term sinus health.
