What Is a Facelift?

Ayala ENT & Facial Plastic Surgery offers several medical spa treatments to delay the aging process, but the procedures cannot stop the hands of time. At some point, Dr. Ayala may recommend a facelift for advanced sagging, creases, folds, and wrinkles. A facelift is a comprehensive surgery that corrects wrinkles in all areas of the face.

Clinically known as a rhytidectomy, a facelift works by repositioning the deeper facial tissues (not just the skin) to restore structure and definition that has softened over time. Modern facelift techniques have come a long way. The concern many patients have about looking “pulled” or overdone is largely a product of older approaches that focused on skin tension alone.

Today, the goal is a result that looks rested and natural, not surgical. According to the American Society of Plastic Surgeons, facelift surgery ranks among the top surgical cosmetic procedures performed in the United States each year, with over 80,000 procedures performed annually.

Facelift Techniques: SMAS, Deep Plane and Preservation Deep Plane

The technique Dr. Ayala chooses depends on a patient’s anatomy, the degree of visible change and where the face has shifted most over time. Three of the most established approaches are the SMAS facelift, the deep plane facelift and the preservation deep plane facelift, each working at a different layer of the facial anatomy.

The SMAS facelift (superficial musculoaponeurotic system) targets the fibromuscular layer beneath the skin rather than the skin alone. By lifting and repositioning this deeper structure, the SMAS technique produces more lasting results than procedures that rely primarily on skin tension. It is a well-evidenced approach that is appropriate for a broad range of candidates.

The deep plane facelift goes a step further, releasing the ligaments that tether the facial tissues in place and allowing the surgeon to reposition the face at a deeper level. This tends to address more advanced volume shift and jowling, particularly in the midface, with results that are generally longer-lasting and less prone to the “pulled” appearance associated with skin-only techniques.

The preservation deep plane facelift is a refinement of the deep plane approach that prioritizes keeping the facial ligaments intact wherever possible, moving tissues along their natural anatomical paths rather than cutting through them. The goal is a result that looks inherently natural.

What Is a Mini Facelift?

A mini facelift may be adequate to help you look younger. It is typically recommended for men and women who have sagging that is limited to the lower third of the face, around the jowls, mouth, and nose.

The incisions in a mini facelift are smaller, and the recovery is generally quicker than a full facelift, making it a reasonable option for patients who are earlier in the aging process and want to address specific concerns without undergoing more extensive surgery. Dr. Ayala evaluates each patient individually to determine whether a mini facelift or a full facelift is the more appropriate approach based on anatomy, skin quality and what the patient wants to achieve.

What Are the Benefits of a Facelift?

  • Appropriate for advanced signs of aging
  • Provides dramatic results
  • May take 10 years or more off your appearance
  • Can be combined with other procedures to improve surface flaws, such as age spots and fine lines

Beyond the physical changes, patients often describe a renewed sense of confidence after facelift surgery, feeling more comfortable in photos, at work and in social settings. Because Dr. Ayala’s approach prioritizes preserving each patient’s natural features, results tend to feel authentic rather than dramatic. As he puts it: “I simplify the beauty in all of us.” His goal is not to give you a new face, but to restore a face that looks even more like you.

Who Is a Good Candidate for a Facelift?

The best candidates for a facelift are men and women who have severe wrinkles, folds, and creases on the entire face or the lower third of the face. The surgery is typically recommended when non-surgical options cannot produce the desired anti-aging benefits.

Good overall health, non-smoking status and realistic expectations are also important factors. Skin that retains some natural elasticity tends to respond particularly well to a facelift procedure. That said, candidacy is always assessed on an individual basis.

Some patients in their late 30s are strong candidates if they have pronounced early changes, while others may benefit more from a combination of surgical and non-surgical approaches. If a facelift is not the right fit for a patient’s goals or anatomy, Dr. Ayala will say so and discuss what would serve them better.

What Is the Procedure for a Facelift?

Dr. Ayala uses several techniques for facelift surgery based on the individual needs of the patient. The primary difference in the techniques is where the incisions are placed, which include:

  • Along the hairline just above the ear following a line in front of the ear to the lobe and behind the ear
  • In the natural crease in front of and behind the ear (may be modified for a simultaneous neck lift)

After making the incisions, Dr. Ayala may suture the facial muscles before tightening the skin and trimming any excess.

The procedure typically takes three to five hours, depending on the extent of treatment and whether additional procedures are being performed at the same time. Most patients go home the same day or after a brief observation period.

In many ways, a facelift is multiple operations in one, each requiring careful attention to the layered anatomy of the face. Dr. Ayala’s background as a double board-certified ENT and facial plastic surgeon, combined with his training at Harvard and his experience as a military trauma surgeon, gives him a thorough understanding of facial anatomy that informs every step of the procedure.

How Is the Vectra H1 Imaging System Used for Facial Plastic Surgery?

Imagining how you will look after facelift surgery is challenging, so Dr. Ayala uses the Vectra H1 imaging system at his practice. The system creates 3D images of how you look now, as well as a virtual representation of how you will look after your facelift.

Dr. Ayala uses the Vectra H1 in all facial plastic surgery consultations. It gives patients and surgeons a shared visual reference during the planning process, grounding the conversation in something concrete rather than abstract descriptions alone. Patients leave their consultation with a clearer sense of what is possible and what to expect, which is something Dr. Ayala considers essential before any surgical commitment is made.

What Is Recovery Like After a Facelift?

You will rest at home for about two weeks after your facelift. Swelling, bruising, fatigue, and tightness are common symptoms. It may take up to one year for the tissues to heal completely. Other potential complications that could arise post-surgery include:

  • Infection
  • Bleeding
  • Scarring
  • Asymmetry
  • Reoperation

The first week tends to involve the most noticeable effects. Keeping the head elevated and avoiding strenuous activity are two of the most important things patients can do during early recovery. By the end of the second week, most patients feel comfortable with public appearances, though some residual swelling may persist for several more weeks. Final results typically come together around three to six months post-op as the tissues settle into their new position.

Dr. Ayala and his team walk patients through every stage of recovery before surgery takes place, so nothing comes as a surprise. Follow-up appointments are an important part of the process and give Dr. Ayala the opportunity to monitor healing and address any questions as they come up.

Can a Facelift Be Paired With Other Procedures?

Yes, a facelift is successfully paired with other procedures when planned and implemented carefully by a skilled surgeon. Many patients who are good facelift candidates also have concerns about the area below the jawline, such as loose or excess skin beneath the chin, vertical neck bands or a general softening of the neck contour. A neck lift is a natural complement to the facelift for this reason. Performing these two procedures together often produces the most cohesive, balanced result.

Dr. Ayala does not automatically recommend combining procedures. As he has said, if he always did the same thing, it wouldn’t be custom. For some patients, a facelift alone accomplishes everything they want. For others, adding a neck lift or a complementary procedure like blepharoplasty makes the outcome more complete. The conversation about what to combine, if anything, always starts with a thorough consultation and an honest assessment of what will genuinely benefit each patient.

How Much Does a Facelift Cost?

After your consultation with Dr. Ayala, we review the cost of your facelift and explain our financing options. For your convenience, Ayala ENT & Facial Plastic Surgery accepts Prosper® and CareCredit®.

The total cost varies depending on the extent of the procedure, whether it is combined with any additional surgeries, anesthesia fees and the surgical facility. Facelift surgery is considered an elective cosmetic procedure and is generally not covered by insurance. Exact pricing is discussed during your consultation once Dr. Ayala has a clear picture of what your procedure will involve.

Frequently Asked Questions About Facelifts

How long do facelift results last?

Facelift results are long-lasting, with most patients enjoying their outcome for 10 years or more. The procedure does not prevent aging, but it prepares you to age more gracefully by addressing several key parts of the process. Lifestyle factors like sun protection, a consistent skincare routine, and not smoking all play a role in how long results are maintained.

Will I look natural after a facelift?

This is one of the most common concerns patients bring to their consultation. Modern facelift techniques address the deeper layers of the face rather than simply pulling the skin taut, which is what produces a natural result. Dr. Ayala’s approach prioritizes maintaining each patient’s inherent features. His philosophy is that his job is to enhance what is already there, not replace it.

What is the difference between a facelift and a neck lift?

A facelift focuses primarily on the cheeks, lower face and jawline. A neck lift targets the area below the jaw, including loose neck skin and platysmal banding. The two procedures are often performed together when a patient has concerns in both areas, as the results tend to complement each other well.

Is a facelift painful?

Discomfort in the first few days after surgery is normal but manageable with medication prescribed by Dr. Ayala. Most patients describe the sensation as tightness and pressure rather than sharp pain. Swelling and bruising are the more noticeable effects in the early days of recovery.

Am I too young (or too old) for a facelift?

There is no hard age rule. Most facelift patients are in their 40s to 60s, but candidacy is based on anatomy, overall health and personal goals rather than age alone. The best way to find out where you stand is a one-on-one consultation with Dr. Ayala.

How do I prepare for a facelift consultation?

Come with a clear sense of what you’d like to address and what kind of outcome you’re hoping for. Be ready to share your medical history, current medications or supplements and any prior surgeries. Dr. Ayala uses the Vectra H1 imaging system during consultations, so you’ll leave with a visual sense of what your results could look like.
