Cheek Augmentation in McAllen, TX

Restore Midface Volume With a Double Board-Certified Facial Plastic Surgeon

Dr. Carlos Ayala is double board certified in otolaryngology and facial plastic surgery and offers cheek augmentation for men and women in all of Texas including McAllen, Mission, Austin, San Antonio, Houston, Corpus Christi, the Rio Grande Valley and South Padre Island.

Your cheeks do more than fill out your face. They create the contours, highlights and soft shadows that give your midface its structure. When cheek volume is full and well-positioned, it draws attention to your eyes, lifts the lower face and makes everything look more balanced. When that volume diminishes over time or was never quite where you wanted it, those subtle shifts can change how your entire face reads.

At Ayala ENT & Facial Plastic Surgery, Dr. Carlos Ayala performs cheek augmentation using dermal fillers, fat transfer and cheek implants. His approach starts with your bone structure and works outward, focusing on where volume will have the most impact without overcorrecting.

What Is Cheek Augmentation?

Cheek augmentation is a cosmetic procedure that restores or adds volume to the midface. It can address flat or hollow cheeks, age-related volume loss and asymmetry. For some patients, the goal is restoring fullness that has changed over the years. For others, it’s about building cheek definition they’ve always wanted.

The midface is one of the first areas to show visible changes as we age. The descent of cheek fat pads and loss of underlying bone support contribute to a flatter, less defined midface over time. Cheek augmentation counteracts those changes and can make the face appear more youthful and proportionate.

Cheek Augmentation Options

Dr. Ayala offers three approaches to cheek augmentation, and the right one depends on how much correction you need, whether you want a temporary or permanent result, and your overall treatment goals.

Cheek Augmentation With Dermal Filler

Injectable fillers like Juvederm and Restylane are the most common way to add cheek volume without surgery. Results are immediate, downtime is minimal and the procedure takes less than 30 minutes. Filler typically lasts several months to over a year, depending on the product and placement.

Cheek Implants

For patients who want a permanent structural change, silicone cheek implant surgery provides lasting projection and definition. Implants are a good fit when there’s a more noticeable volume deficit or if you prefer a one-time procedure over repeated filler appointments.

Fat Transfer to the Cheeks

Fat transfer (fat grafting) is a minimally invasive surgical option that uses your own fat to restore cheek volume. The fat is harvested from a donor area, typically the abdomen, and then processed and refined into a smooth, injectable form. Because it uses your own tissue, fat transfer offers a biocompatible alternative to synthetic fillers or implants.

Dr. Ayala’s Approach to Natural-Looking Cheek Enhancement

What separates a good cheek augmentation from a noticeable one often comes down to technique and restraint. Dr. Ayala’s philosophy is rooted in structure, not overfilling. He assesses each patient’s underlying bone structure and uses a deep injection technique, placing filler closer to the cheekbone rather than superficially. This creates a more natural lift and support instead of a puffy or overdone look.

He also evaluates the full face during every consultation. Temporal hollowing, for example, is a subtle change that can age the face considerably. Restoring volume there with filler can smooth the contour of the upper face and complement cheek enhancement. Equally important is knowing when to hold back. Dr. Ayala is conservative with the jawline and avoids filling areas that would widen the face or create an unnatural shape. In some cases, he may recommend enhancing the chin to improve overall harmony without over-treating the cheeks.

Am I a Good Candidate for Cheek Augmentation?

Most adults in good general health are candidates for cheek augmentation. You may be a good fit if you’ve noticed a loss of midface volume over time, your cheeks appear flat or hollow, or you’d like more defined cheek contours.

Good candidates for cheek augmentation have realistic expectations about what the procedure can accomplish. During your consultation, Dr. Ayala will evaluate your facial structure, discuss what you’re looking to change and recommend the approach that makes the most sense for your anatomy.

You may not be an ideal candidate at this time if you have an active infection in the treatment area, you are pregnant or breastfeeding, or you have certain medical conditions that increase surgical risk.

What to Expect During a Cheek Augmentation Procedure

Dermal Filler Injections

Cheek filler is an in-office treatment. After applying a topical numbing agent, Dr. Ayala injects filler at precise depths along the cheekbone and midface to add volume and improve contour. The entire appointment typically takes 30 minutes, and you can return to your normal routine the same day.

Cheek Implant Surgery

Cheek implant surgery is performed under local anesthesia with sedation or general anesthesia. Dr. Ayala places the implants through small incisions inside the mouth, positioning them over the cheekbones to add projection where it’s needed. The incisions are closed with sutures that dissolve, leaving no visible scars. The procedure usually takes one to two hours.

Cheek Fat Transfer

Fat transfer to the cheeks begins with harvesting fat from a donor site, typically the abdomen, using a small cannula inserted through a tiny incision. The fat is then processed, purified and injected into the midface in small amounts, layered to create a smooth, even result. Because some transferred fat is naturally reabsorbed by the body, Dr. Ayala may slightly overcorrect during the initial procedure. The process takes one to two hours depending on whether other areas are being treated at the same time.

What Is Cheek Augmentation Recovery Like?

Recovery varies depending on the method used.

Dermal filler requires no real downtime. Minor swelling or tenderness at the injection sites is common and usually resolves within a day or two.

Cheek implants involve a recovery similar to other facial surgeries. Expect swelling and tightness in the midface for the first week, with most patients returning to work and daily activities within seven to 10 days. Swelling continues to improve over the following weeks, and final results are typically visible within two to three months.

Fat transfer recovery falls somewhere in between. You’ll have mild swelling in the cheeks and some soreness at the donor site. Most fat grafting patients feel comfortable resuming daily activities within a week. The transferred fat takes several weeks to settle, and the final volume becomes apparent as swelling resolves over the first two to three months.

Dr. Ayala monitors all surgical patients at follow-up appointments to make sure healing is progressing well.

Frequently Asked Questions

What results can I expect from cheek augmentation?

Results depend on the method. Dermal filler produces immediate improvement that lasts several months to over a year. Cheek implants are permanent and do not need replacement. Fat transfer results are long-lasting, though some volume may be reabsorbed in the weeks following the procedure. Across all three options, Dr. Ayala’s goal is a result that looks refreshed and proportionate, never overdone.

What about buccal fat removal?

Buccal fat removal is a different procedure. Rather than adding volume to the cheeks, it removes fat from the lower cheek area to create a more sculpted, contoured look. It’s typically a better fit for younger patients with naturally full, round cheeks who want more definition. Dr. Ayala can discuss whether buccal fat removal, cheek augmentation or a combination makes sense for your anatomy during your consultation. You can also read more in our blog post on considerations for buccal fat removal.

Can cheek augmentation be combined with other procedures?

Yes. Cheek augmentation pairs well with other procedures like facelift surgery, eyelid surgery, chin augmentation, neck lift or rhinoplasty for more comprehensive facial rejuvenation and balancing. Dr. Ayala also offers PRP therapy as a regenerative option and other med spa services to improve skin quality and stimulate collagen in the treated area.

What affects the cost of cheek augmentation?

Cheek augmentation cost varies depending on the approach (filler, implants or fat transfer) and whether the procedure is combined with other treatments. Dr. Ayala’s team will provide a detailed quote at the time of your consultation. Financing is available through Prosper and CareCredit, and the practice also offers an in-office layaway plan with a minimum deposit of $500 to secure your procedure date.
