What Is a Brow Lift?

A brow lift, also called a forehead lift, is a surgical procedure that raises the brows and smooths the forehead to reverse the visible effects of aging in the upper third of the face. Once the skin has lost the collagen and elastin that kept it firm, gravity can have more of an effect, pulling the soft tissue of the brow downward and settling it into a lower position. A lowered brow can crowd the eyes and deepen the lines across the forehead. Because the brow and the upper eyelids are closely related, Dr. Ayala evaluates both areas together when planning your treatment.

What Can a Brow Lift Address?

A brow lift can correct concerns that injectables and topical treatments cannot fully resolve. For example, while neuromodulators can prevent dynamic wrinkles due to muscle movement, they cannot address excess skin. A brow lift can address:

  • A low or sagging brow line
  • Heaviness or hooding of the upper eyelids caused by a descended brow
  • Deep horizontal creases across the forehead
  • Vertical frown lines that form between the brows
  • Brows that sit unevenly or asymmetrically
  • A tired or angry resting expression due to a descended brow

A brow lift pairs well with eyelid surgery or a facelift for a balanced result across the whole face. Dr. Ayala will help determine whether brow lift surgery on its own is the right path for you or whether you may benefit from nonsurgical treatments or a combination of surgeries.

Who Is a Good Candidate for a Brow Lift?

Good candidates for a brow lift are healthy adults whose foreheads and brows show clear signs of aging that have moved beyond what nonsurgical options can treat. Dr. Ayala makes sure that each brow lift candidate understands what results this surgery can offer as well as its limitations. If you smoke, you will be asked to stop well before the surgery, since nicotine interferes with healing. You will also be screened for medical conditions that may affect your candidacy. You should be in good general health without skin infections or other contraindications.

Brow Lift Techniques at Ayala ENT & Facial Plastic Surgery

Dr. Ayala chooses the brow lift technique that he uses based on your individual situation. This choice is made considering factors like the shape of your face, the degree of brow descent, the area that most needs lifting and what you want from your results. The two approaches he uses most are the direct brow lift and the lateral brow lift.

Direct Brow Lift

A direct brow lift provides control over how much the brow is raised and the balance of the two sides. In this method, the incision is placed just above the brow, which allows Dr. Ayala to remove a measured amount of skin and lift the brow into its new position. This technique is used for patients with heavier brows or more pronounced brow ptosis. Because this method offers so much control, it can be used in cases that are too difficult for other techniques to handle.

Lateral Brow Lift

A lateral brow lift focuses on the outer portion of the brow, which is usually the first area to descend with age because the frontalis muscle doesn’t extend to this area. Elevating the tail of the brow opens the outer corner of the eye and reduces early signs of aging. Dr. Ayala uses this option for patients who won’t need as much correction in the center of the brow to see a more refreshed and youthful appearance.

What Happens During Brow Lift Surgery?

Brow lift surgery at Ayala ENT & Facial Plastic Surgery is performed at an accredited surgical facility under general anesthesia or local anesthesia with IV sedation. Most procedures take one to two hours, though this timing will depend on the technique used and whether any additional work is being done at the same time.

Once you are comfortable and the anesthesia has taken effect, Dr. Ayala will make the incision, either just above the brow for a direct lift or near the tail of the brow for a lateral lift. He then lifts and repositions the tissue, removing excess skin. The underlying muscles can also be adjusted to improve the lift. Once the lift has been performed, incisions are closed to keep any scarring discreet and a light dressing is applied to protect the area as it heals.

What Is Brow Lift Recovery Like?

Your recovery will depend on the technique and the extent of your procedure. Usually, a lateral lift involves a shorter downtime. A typical healing timeline often looks like this:

  • Days 1 to 10: Expect swelling, bruising and a feeling of tightness across the forehead. Keeping your head elevated helps the swelling settle more quickly.
  • Weeks 1 to 2: Sutures are typically removed during this window, and the worst of the bruising fades. Many patients return to desk work and gentle daily routines.
  • Weeks 3 to 6: Most of the visible swelling has resolved, and the lifted brow position becomes easier to see. Strenuous exercise should still wait.
  • Months 3 to 6: Lingering swelling continues to fade, and your result looks increasingly natural and settled.
  • 6 to 12 months: Healing is complete, and your final result is fully apparent.

Dr. Ayala will follow your recovery closely with scheduled visits that are included as part of your surgical plan.

Why Choose Dr. Ayala for Your Brow Lift?

The upper face is one of the most expressive areas of the body, and small changes can make a big difference. Getting the results that you are looking for comes from finding a surgeon who understands the nuances of facial anatomy and how a face moves and reacts after surgery. Dr. Ayala brings this background. He earned his medical degree from UCLA School of Medicine, graduating with honors, and then completed a residency in otolaryngology at Harvard Medical School. He went on to a fellowship in facial plastic and reconstructive surgery at the Lasky Clinic in Beverly Hills. He is the first double board-certified otolaryngologist and facial plastic surgeon in the Rio Grande Valley.

During his service at Bagram Airfield in Afghanistan, Dr. Ayala reconstructed the faces of soldiers and civilians who had suffered severe trauma, work that demanded a level of precision and judgment few elective practices ever require. He brings this experience to every procedure as well as a level of compassion and a heart for service that can be difficult to find anywhere else.

Frequently Asked Questions About Brow Lift Surgery

Will a brow lift help my droopy eyelids?

Sometimes the heaviness you notice on your upper eyelids comes from a brow that has dropped rather than from the eyelid itself. In those cases, a brow lift may be a better option. Dr. Ayala will look at both areas during your consultation and let you know which surgery will be more beneficial. Sometimes a combination surgery will deliver the best results.

How long does a brow lift last?

A brow lift produces long-lasting results, and many patients enjoy their outcome for a decade or more. Remember that your face will continue to age naturally, including changes in skin texture, bone density and fat pads. Caring for your skin and avoiding tobacco products can help your results last longer.

What is the difference between a brow lift and Botox®?

Botox relaxes the muscles that create forehead lines and can give the brow a slight lift. This effect is temporary and limited. When the brow has descended significantly, only surgery can reposition the tissue and hold it in place. A brow lift may be the right choice once injectables no longer give you the result you are looking for.

What are the risks of a brow lift?

Every surgery carries some risk, including brow lift surgery. Possible complications include bruising, swelling, bleeding, infection, scarring, asymmetry and changes in scalp sensation. The best way to minimize risk is to choose an experienced surgeon with the proper certifications and to closely follow post-operative instructions.

How much does a brow lift cost?

Cost varies from patient to patient based on factors like the specific technique and whether the brow lift is being combined with other procedures, like an eyelid lift or facelift. A lateral browlift costs around $5,000. You will receive a personalized quote after your consultation so you can begin the planning process. Financing is available through Prosper® and CareCredit®, along with an in-office layaway plan that lets you make flexible payments toward your surgery date.
