Functional Rhinoplasty in McAllen, TX

Dr. Carlos Ayala, The Nose Expert™

Dr. Carlos Ayala is double board certified in otolaryngology and facial plastic surgery and offers functional rhinoplasty (nasal reconstruction) for men and women in all of Texas including McAllen, Mission, Austin, San Antonio, Houston, Corpus Christi, the Rio Grande Valley and South Padre Island.

For most people, breathing is something that happens without any thought. But when that process becomes restricted or labored by nasal obstructions, it affects everything. Your sleep suffers, your energy drops, and even simple activities feel harder than they should.

Functional rhinoplasty addresses the structural reasons behind those problems. At Ayala ENT & Facial Plastic Surgery, Dr. Carlos Ayala brings a perspective most surgeons can’t offer: he trained as both an otolaryngologist (ear, nose and throat specialist) and a facial plastic surgeon, which means he understands the nose from every angle, inside and out.

What Is Functional Rhinoplasty?

Functional rhinoplasty is a surgical procedure that corrects structural problems inside the nose that interfere with normal breathing. Unlike cosmetic rhinoplasty, which focuses on how the nose looks from the outside, functional rhinoplasty targets the underlying anatomy: the septum, the turbinates, the nasal valves and other internal structures that regulate airflow.

That said, the two goals are not always entirely separate. In some cases, correcting a structural problem can also refine the appearance of the nose. Similarly, patients who want to address cosmetic concerns alongside airflow issues may be good candidates for a combined procedure.

What Conditions Does Functional Rhinoplasty Treat?

Chronic breathing difficulty can stem from several different structural causes, and identifying the right one is the first step. Dr. Ayala evaluates each patient individually to determine what is actually limiting airflow before recommending a surgical approach.

Functional rhinoplasty can address:

  • A deviated septum, where the wall dividing the nasal passages sits off-center and restricts one or both sides
  • Enlarged turbinates, the bony structures lining the nasal passages that can swell and block airflow
  • Nasal valve collapse, which affects the narrowest part of the nasal passage and is a common but frequently missed cause of chronic obstruction
  • Nasal polyps, which are soft growths that develop along the sinus lining and can significantly reduce airflow
  • Structural damage from injury or trauma that affects breathing
  • Breathing problems that developed or persisted after a previous rhinoplasty

Depending on what Dr. Ayala finds during your evaluation, he may recommend septoplasty, turbinoplasty, nasal obstruction surgery or a combination of techniques. If your symptoms are being driven by sinus blockage, chronic inflammation or another ENT condition, he may recommend a different treatment, such as balloon sinuplasty, when appropriate.

What Is Nasal Reconstruction?

Nasal reconstruction is a specific category of functional rhinoplasty for patients whose nasal structures have been damaged or lost due to trauma, cancer treatment, injury or a previous surgery. The goal is to rebuild the internal framework so the nose can function properly again.

This type of work requires a surgeon with genuine reconstructive experience. Dr. Ayala spent time as a facial plastic surgeon at Bagram Airfield in Afghanistan, where he performed complex nasal and facial reconstructions on soldiers and civilians with severe trauma. That background, combined with his dual board certification in otolaryngology and facial plastic surgery, gives him the expertise needed for both minor and complex reconstructive cases.

How Do I Know If My Airflow Is Restricted?

When you’ve been breathing a certain way your entire life, the idea that something could be structurally off isn’t always obvious, because you have nothing else to compare it to. If you think your nasal breathing might be restricted, some signs to look for include:

  • You frequently breathe through your mouth, especially during sleep or exercise
  • You wake up with a dry mouth or a sore throat in the morning
  • Sleep feels light or unrestful and you can’t point to an obvious reason
  • Congestion seems to linger even when you’re not sick and allergies have been ruled out
  • You’ve been told you snore, but you’ve never been diagnosed with sleep apnea
  • You experience facial pain or pressure, especially around the cheeks and eyes
  • You feel like you aren’t getting enough air when breathing through your nose

Longstanding nasal breathing issues are worth evaluating, even if you have learned to live with them. An evaluation with Dr. Ayala can identify the underlying cause of your symptoms and determine whether surgery could help you breathe more comfortably.

What to Expect During Functional Rhinoplasty

Functional rhinoplasty at Ayala ENT & Facial Plastic Surgery is performed under general anesthesia. For certain procedures, such as the Simply Sinus Procedure™, Dr. Ayala offers an in-office option under local anesthesia. Procedure time varies depending on the complexity of the work involved and whether multiple structural issues are being addressed at once.

During surgery, Dr. Ayala accesses the nasal structures through incisions on the inside of the nose and across the columella, the narrow strip of skin that separates the nostrils. For most functional cases, an open technique is used to provide better access to the internal nasal framework. From there, the approach varies based on what needs to be corrected:

  • A deviated septum is straightened and repositioned to improve airflow
  • Enlarged turbinates are reduced to open the airway
  • A collapsed nasal valve may be supported with cartilage grafts

When cosmetic concerns are addressed alongside functional ones, Dr. Ayala integrates both aspects into a single procedure, refining the size and shape of the nose to improve its overall appearance. Patients are left with a nose that breathes well and looks proportional.

What Is Recovery Like After Functional Rhinoplasty?

Functional and cosmetic rhinoplasties follow a similar recovery timeline. You can expect discomfort, swelling and bruising that peak within 48-72 hours of surgery. While it can take up to a year for swelling to fully resolve, most patients feel meaningfully better within the first few weeks, with ongoing improvement as pain and swelling continue to decrease.

Here is what a typical recovery timeline looks like for most patients:

  • Days 1 to 7: Discomfort, congestion, swelling and drainage are normal. Nasal splints or packing will be in place for support, depending on the technique used.
  • Days 7 to 14: Splints are removed at the end of week one. Congestion begins to ease, and many patients return to desk work and light daily activities.
  • Weeks 3 to 6: Breathing improvement becomes more noticeable. Strenuous activity, contact sports and anything that could impact the nose should still be avoided.
  • Months 2 to 4: Most patients are fully back to their regular routine and noticing consistent improvement in airflow.
  • 6 months and beyond: Final results are visible as residual swelling fully resolves.

Like all rhinoplasty patients, those who have functional surgery are seen at regular follow-up appointments, which are included at no additional cost. Dr. Ayala checks in at one week, one month, three months and six months after surgery to make sure healing is progressing well.

Potential complications, though uncommon when surgery is performed by a skilled surgeon, may include bruising or facial swelling, infection, bleeding, scarring, asymmetry, septal perforation, cerebrospinal fluid (CSF) leak or the need for reoperation. Dr. Ayala discusses all risks and expectations with you thoroughly during your consultation.

Why Patients Choose Dr. Ayala for Functional Rhinoplasty

There are surgeons who fix breathing problems, and there are surgeons who address how the nose looks. Dr. Ayala does both, and his dual training ensures neither side of the equation is compromised.

Dr. Ayala is the first double board-certified otolaryngologist and facial plastic surgeon in the Rio Grande Valley. His background includes an otolaryngologist residency at Harvard and a facial plastic surgery fellowship in Beverly Hills. Before building his practice in McAllen, he served as a military surgeon abroad for over two decades, reconstructing faces and nasal passages in conditions where there was no room for anything less than precision.

For years, patients in the Rio Grande Valley have often looked to larger cities for highly specialized nasal surgery. Dr. Ayala brings that level of functional and aesthetic expertise closer to home, giving patients access to advanced rhinoplasty care right here in South Texas.

Frequently Asked Questions

What is the difference between functional and cosmetic rhinoplasty?

Cosmetic rhinoplasty changes the outward shape or size of the nose. Functional rhinoplasty corrects structural problems that interfere with breathing. The two can be performed together, but they address different things, and insurance may cover the functional portion of a combined procedure when medical necessity is established.

Is functional rhinoplasty covered by insurance?

In many cases, yes. When functional rhinoplasty is performed to correct a documented structural problem, such as a deviated septum or enlarged turbinates that cause chronic breathing obstruction, insurance may cover part or all of the cost. Dr. Ayala’s team can help you understand your coverage before surgery.

How long does it take to notice improvement in breathing?

Many patients notice improved airflow a few weeks after surgery, once the initial swelling and congestion begin to resolve. Full results take longer to develop, particularly if cartilage grafts or more involved reconstruction were part of your surgical plan. Most patients experience meaningful, lasting improvement by the two- to three-month mark.

What is the Simply Sinus Procedure™?

The Simply Sinus Procedure™ is Dr. Ayala’s trademarked approach to balloon sinuplasty, performed in-office with local anesthesia. It is designed for patients with chronic sinus congestion who want relief without traditional sinus surgery. The procedure opens blocked sinus passages and restores normal drainage without removing bone or tissue.

Can functional rhinoplasty help with snoring?

It can, depending on the cause. When snoring is linked to restricted nasal airflow, correcting the underlying structural obstruction often improves it. However, snoring has multiple possible causes, and Dr. Ayala will evaluate whether nasal obstruction is actually contributing to yours before recommending surgery.

How much does functional rhinoplasty cost?

Cost varies based on the specific procedures involved and the complexity of the case. We can provide a detailed quote at the time of your consultation. Financing is available through Prosper® and CareCredit®, and we also offer an in-office layaway plan with a minimum deposit of $500 to secure your surgery date. The remaining balance is due in full at least two weeks before your procedure.
