Most often it means adding volume, but the term also covers reshaping the lip line, softening fine lines around the mouth and, in some cases, reducing lips that have become too full. Most patients who come in for fuller lips are candidates for lip augmentation with dermal fillers. Dr. Ayala uses hyaluronic acid products such as Juvéderm Volbella and Restylane Silk, both formulated for the delicate tissue of the lips. Hyaluronic acid occurs naturally in the body, which is part of why these fillers feel soft and integrate well. Results are not permanent, and that turns out to be an advantage. You can refine the shape at follow-up visits, and if you ever want the filler gone, it can be dissolved.
A surgical lip lift suits patients who want a lasting result without maintaining filler over time. By removing a small amount of skin below the nose, the procedure shortens the space between the nose and upper lip, rolls the lip outward slightly and shows a touch more of the upper teeth. The effect is a more youthful, balanced mouth that holds its shape for years. Dr. Ayala generally points patients toward surgery when they are after long-term or corrective results that injectables cannot deliver.
Lip reduction addresses the opposite concern: lips that carry too much volume. This often comes up with patients who have a history of filler that never fully dissolved, which can leave excess fullness that further injections will not fix. By removing a thin strip of tissue from inside the lip, Dr. Ayala brings the lips back into proportion with the rest of the face.
If your lips have always been on the thinner side or have lost definition over time, filler is usually a comfortable starting point. Good candidates are in reasonable health and have a clear sense of the change they want. Surgical options tend to fit patients looking for permanence or correction rather than something they refresh every several months.
Be open about your history with injectables during the consultation. If you have had filler before, especially filler that left you unhappy or that has not gone away, that information shapes the plan. It can be the difference between recommending more filler and recommending a reduction.

Filler lip injection treatments are quick and done in the office. Rather than a standard needle, Dr. Ayala places filler with a cannula, a thin, blunt-tipped tube he guides through a single tiny entry point made with a needle. He uses a high-quality SoftFil cannula for this. The technique lowers the chance of bruising and of nicking the small vessels in the lips, spreads the gel more evenly and gives him precise control over where volume lands. Most filler appointments wrap up in well under an hour.
The surgical lip lift and lip reduction are performed under local anesthesia. For a lip lift, the incision sits in the natural crease at the base of the nose, where it heals discreetly. For a reduction, the incision is hidden on the inside of the lip. Both are outpatient procedures, so you go home the same day.
Filler requires little to no downtime. Your lips may feel tight for a day or two, and minor swelling or a small bruise may appear at the entry point before fading quickly. You can return to your day right away.
Surgical recovery asks for more patience. Expect swelling and some tenderness in the first several days, with most patients back to normal activities within about a week. As with any procedure, possible complications include bruising, swelling, infection, bleeding, asymmetry and, with surgery, scarring. Dr. Ayala reviews aftercare in detail so you know what is normal and what is not.

Discomfort is minimal. A topical numbing agent is applied first, and the cannula enters through a single small opening, which is gentler than multiple needle sticks.
Yes. Because hyaluronic acid fillers are dissolvable, unwanted or excess filler can be broken down with an enzyme injection. Older filler that has hardened or will not dissolve may be better addressed with lip reduction.
